Heat and Sauna Therapy

Sauna is often viewed simply as a place to relax.  Physiologically, however, it creates a controlled thermal challenge.

As the body warms, blood flow shifts toward the skin, heart rate rises, sweating increases, and cardiovascular and cellular stress responses become active. Once the exposure ends, cooling and recovery begin.  The potential value of heat therapy may therefore lie not only in the heat itself, but in how the body responds, recovers, and potentially adapts over time.

Heat as a Controlled Physiologic Stressor

The body maintains its internal temperature through a process called thermoregulation.

As skin and core temperatures rise, the hypothalamus—an area of the brain involved in temperature control—initiates several cooling responses:

  • Blood vessels near the skin widen.

  • Blood flow is redirected toward the body’s surface.

  • Heart rate and cardiac output may increase.

  • Sweating promotes evaporative cooling.

  • Fluid and electrolytes are lost.

These changes create a temporary but meaningful physiologic workload.  The intensity of that workload depends on the temperature, humidity, radiant heat, duration, sauna type, hydration status, medications, medical conditions, and individual heat tolerance.[1,2]

Heat exposure → thermoregulatory response → cardiovascular and cellular signaling → cooling and recovery → potential adaptation

Cardiovascular response

As blood vessels near the skin widen, the heart circulates more blood toward the body’s surface to release heat.  Heart rate generally rises, and blood pressure may increase, remain stable, or decline depending on the person and the phase of exposure.  Blood pressure may remain lower temporarily during the post-sauna recovery period.[1,3]

Because sauna increases heart rate and circulatory demand, it is sometimes compared with low-to-moderate cardiovascular exercise however that comparison has limitations. 

Sauna does not reproduce the muscular contraction, mechanical loading, energy expenditure, coordination, or complete cardiorespiratory adaptations produced by exercise. It should complement—not replace—aerobic exercise, strength training, mobility, and regular movement.

Heat-shock proteins and cellular stress

Heat exposure can activate heat-shock proteins, which help cells stabilize, repair, and correctly fold other proteins during periods of stress.

This response is scientifically interesting because maintaining protein quality is important to cellular function and healthy aging.  However, heat-shock signaling does not prove that sauna reverses aging, activates a universal “longevity pathway,” or treats disease.  The long-term clinical significance of these cellular responses remains under investigation.[4]

Vascular signaling

The endothelium is the thin cellular lining inside blood vessels.  As heat increases blood flow, the resulting movement of blood along the vessel wall creates shear stress.

This mechanical signal may influence endothelial function and nitric-oxide-related pathways, which help blood vessels relax.  Studies have reported improvements in selected vascular and blood-pressure measures, particularly when sauna accompanies exercise.[5]

However, the overall evidence is mixed.  A 2025 systematic review of 20 randomized trials found no significant pooled improvement in most cardiometabolic and vascular outcomes. A possible reduction in systolic blood pressure appeared in certain subgroups, but study designs and results varied considerably.[2]

Autonomic recovery and relaxation

Heat initially activates a physiologic stress response.  Heart rate rises, sweating intensifies, and the body works to maintain circulation and temperature control.

During cooling and post-sauna rest, some people experience relaxation or autonomic “downshifting.” Researchers have investigated changes in parasympathetic activity and heart-rate variability, but the longer-term evidence remains inconsistent.[2]

Not everyone responds identically. Timing, temperature, hydration, expectations, and individual tolerance can determine whether a session feels calming, stimulating, or exhausting.

Why Repeated Exposure May Matter

One sauna session creates an acute response.  Repeated, appropriately recovered exposures may help the body become more efficient at managing future heat.

This is related to hormesis: the concept that a manageable stressor may stimulate an adaptive response. With repeated heat exposure, researchers have examined changes in sweating efficiency, skin blood flow, plasma volume, heat tolerance, and cardiovascular strain.[6]

The key principle is that more stress is not automatically better.  Too little exposure may provide a limited stimulus, while excessive heat may increase dehydration and physiologic strain without producing additional benefit.

Consistency, dose, recovery, and individual tolerance matter more than pursuing the highest possible temperature.

Major Finnish cohort studies have reported associations between frequent traditional sauna bathing and lower rates of fatal cardiovascular events, all-cause mortality, dementia, and Alzheimer’s disease.[7,8]

These findings are important, but they do not prove that sauna caused the lower risk.

Frequent sauna users may differ from less frequent users in physical activity, baseline health, socioeconomic status, social connection, alcohol use, diet, and other behaviors. Many influential studies also involved middle-aged Finnish men and specific traditional-sauna practices.

The findings cannot automatically be generalized to every population, sauna type, temperature, or pattern of use. They should be viewed as meaningful, hypothesis-supporting evidence—not definitive proof that sauna prevents disease or extends lifespan.

Traditional and Infrared Saunas Are Not Interchangeable

Traditional saunas primarily heat the surrounding air, which then heats the body.  Infrared saunas deliver radiant energy that is absorbed by the body and surrounding surfaces.

Both can create meaningful thermal stress, but their air temperature, radiant heat, humidity, and user experience differ.  Temperature settings should not be compared as though the environments are physiologically identical.

There is no universal winner. The appropriate choice depends on heat tolerance, desired experience, available space, engineering quality, materials, maintenance requirements, and intended use.

The Dose Is More Than the Temperature

A heat-therapy dose is influenced by:

  • Sauna type and thermal environment

  • Air temperature, radiant heat, and humidity

  • Session duration and frequency

  • Age, fitness, body composition, and heat acclimation

  • Hydration and electrolyte status

  • Medications and medical conditions

  • Recent exercise, illness, alcohol exposure, and recovery

There is no single temperature or session length appropriate for everyone. An effective experience should create a tolerable challenge followed by adequate cooling and recovery—not simply the greatest amount of heat a person can endure.

Sauna can provide a restorative ritual and a controlled source of thermal stress.  It may complement cardiovascular conditioning, encourage relaxation, and support heat adaptation in certain settings.

It remains an adjunct to—not a replacement for—exercise, sleep, nutrition, hydration, stress management, and appropriate medical care.

Facility-based sessions can be valuable, but travel, scheduling, memberships, and repeated fees may create barriers to consistency. A thoughtfully selected home sauna can make a heat-therapy practice more convenient, private, and sustainable.

Product selection should extend beyond appearance or maximum temperature. Engineering, thermal environment, materials, ventilation, safety features, maintenance, available space, and intended use all influence the ownership experience.
